Orthodontic intervention, with braces and other dental appliances, is common for young adolescents and teenagers. Although typically not the case, placement of orthodontic hardware should mean encouraged parental assistance to ensure thorough brushing, increased flossing and regular routine recare visits. 

Being proactive, especially while in brackets, can help to reduce the chance of tooth decay, gum disease or inflammation and a non-esthetically appealing smile due to enamel demineralization. Enamel demineralization, otherwise known as white spot lesions, is a result of inadequate plaque removal combined with an acidic diet. Demineralization, which is the early start of a cavity, can be noticeable at the gumline or almost imprinted on a tooth or multiple teeth once brackets are removed. Dependent on specific patient needs, a more frequent recare may be recommended (every 3-4 months instead of every 6) and a special mouth rinse and/or an extra strength fluoride toothpaste may be prescribed while a patient is in braces.

There are multiple specialized or adjunct tools that can be used to help with more adequate plaque and food debris removal during intervention. Remember, adjunct doesn’t mean replace. Tools such as a Waterpik are great for irrigation around wires and appliances. But, a Waterpik should never be used in replacement of standard brushing and flossing and should always be used in conjunction to. Electronic toothbrushes are also great, but adequacy is based on patient preference as some patients do not like the vibrations. We also find utilizing a magnified makeup mirror to increase visualization can improve home care.

Besides good oral hygiene, a healthy diet is also important during orthodontic intervention. Encouraging foods low in acid and sugar can help decrease the chance of dental decay; specifically interproximal decay. Also, avoiding gummy, sticky snacks can help to minimize frequent trips to the orthodontist due to broken wire and brackets.
